Program Description 

At BAYS Community Care (BCC), we recognize the profound challenges that children and youth face, especially those exhibiting behaviors that may lead to involvement with youth systems. Our mission is to provide compassionate, comprehensive therapeutic interventions that not only address these behaviors but also empower our clients and their families to build resilience and thrive. 

Understanding the diverse circumstances of the families we serve, our services are delivered in various settings, including clinics, homes, schools, child welfare facilities, probation and parole offices, and mental health centers. This flexibility ensures that support is accessible and meets families where they are most comfortable. 

Our overarching goal is to effect lasting behavioral change by enhancing protective factors and mitigating risk factors that impact the family unit. We are committed to valuing every client, aiming to make apositive impact not only on the individuals and families we serve but also on the broader communities. By addressing mental and behavioral health concerns with empathy and expertise, we strive to foster healthier, more resilient communities.

Program Goals

1.       We aim to strengthen the inherent resilience within each youth and family by fostering skills and behaviors that promote well-being and reduce vulnerability to negative influences.

2.      Through targeted interventions, we work to identify and mitigate elements that contribute to behavioral and mental health challenges, thereby decreasing the likelihood of involvement with the juvenile justice or child welfare systems.

3.      Our offerings encompass individual, group, and family counseling, delivered by licensed and master's level clinicians. These services are designed to address specific behavioral and mental health needs, promoting personal growth and family cohesion.

4.      Recognizing the diverse circumstances of the families we serve, we provide counseling in various settings, including clinics, homes, schools, child welfare facilities, probation and parole offices, and mental health centers, ensuring support is available where and when is needed.

5.      We actively engage with local organizations to create a network of support, enhancing the effectiveness of our interventions and contributing to the overall health and safety of the communities we serve.

6.      By focusing on both immediate concerns and long-term outcomes, we strive to instill lasting behavioral changes that empower youth and families to navigate future challenges independently.

 

Provision of Services 

· We offer personalized sessions tailored to each child's unique needs, fostering

  self-awareness and personal growth.

· Our group sessions provide a supportive environment where peers can share experiences,

   learn from one another and develop essential social skills.

· Recognizing the family's pivotal role, we facilitate sessions that strengthen familial bonds,

   improve communication and promote collective healing.

· We focus on children and youth exhibiting behaviors that may lead to involvement with the

  juvenile justice system, as well as those already engaged in such systems.

· Our interventions are grounded in the latest research and best practices, ensuring effective

  and compassionate care.

· We aim to enhance protective factors and reduce risk factors, fostering resilience and

  well-being.

· Our team comprises of Licensed and Master's Level qualified professionals dedicated to

  delivering high-quality care.

· We collaborate with families to develop personalized plans that address their unique

  needs and aspirations.

Population Served

At BAYS Community Care (BCC), we are dedicated to supporting youth and families facing mental health challenges, particularly those at risk of entering the child welfare or juvenile justice systems, at-risk for out of home placement, a factor affecting the youth’s safety and placement status, and are eligible for Medicaid. We understand that each individual and family is unique, and we approach every situation with compassion, respect, and a commitment to fostering resilience. 

Our services are designed to meet the diverse needs of those we serve, including: 

·      Youth exhibiting antisocial behaviors

·      Individuals facing behavioral and/or mental health challenges

·      Youth experiencing aggression or conduct disorders

·      Individuals dealing with mood disorders

·      Students referred for behavioral concerns in school

·      Youth struggling with truancy or at-risk of dropping out

·      Families experiencing conflict

 

Admission Criteria: (included but not limited to)

To be eligible for our program, applicants should meet the following criteria:

·       Youth and families aged 6 and above.

·       Reside within the state of Florida.

·       At least one adult member of the household must be willing to actively

       participate in program services.

·       Eligible for Medicaid

·       Behavioral and Mental Health Indicators:

        - Antisocialbehaviors.

         - Aggressive tendencies or conduct disorders.

         - Mood disorders.

         - School-related behavioral issues.

         -  Truancy or risk of dropping out.

         -  Family conflicts.

·       Risk Factors:

         -  At risk of involvement with the child welfare or juvenile justice systems.

         -  Facing potential out-of-home placement.

         -  Experiencing factors that compromise their safety or placement stability.

 

 

Duration

The duration of services at BAYS Community Care (BCC) is tailored to the unique needs and progress of each youth and family.  The emphasis on treatment planning and the goal of fostering lasting behavioral change allow for an individualized approach to support services. Factors influencing the duration include the complexity of the challenges faced, the specific goals established in collaboration with the family, and the individual's responsiveness to therapeutic interventions. This personalized approach ensures that each client receives the appropriate level of support necessary to achieve meaningful and enduring outcomes.

 

Transition from Program

Transitioning from BAYS Community Care (BCC) is a compassionate and personalized process, designed to honor the unique journey of each youth and family. Our goal is to ensure that the resilience and positive changes cultivated during the program are sustained, empowering families to continue thriving independently.

 As youth and families near their established goals, our dedicated clinicians engage in collaborative discharge planning. This involves working closely with the youth and their families to develop a comprehensive plan that outlines strategies to maintain progress and address potential future challenges. By involving families in this process, we honor their expertise and ensure the plan aligns with their unique strengths and needs.

 Recognizing the importance of ongoing support, we facilitate connections to community resources that can provide continued assistance. This may include referrals to local counseling centers, support groups, educational programs, or other services that align with the family's aspirations and cultural values. By building a network of support, we aim to reinforce the resilience of both the youth and their family.

 To ensure a smooth transition, BCC offers aftercare services such as follow-up sessions and periodic check-ins. These touchpoints provide an opportunity to celebrate successes, address any emerging concerns, and reinforce the skills and strategies learned during the program. Our commitment to compassionate care extends beyond the program, as we remain a steadfast partner in the family's journey.
